A Clear Picture of Your Health
At HealthHub Taree, our doctors and nurses provide comprehensive health assessments and care plans for patients in Taree, Harrington and across the Manning Valley, taking a thorough look at your current health, risk factors and family history in one dedicated appointment. Assessments are often completed with support from our practice nurses, who gather information and take measurements before your doctor completes the clinical review.
Unlike a standard consultation focused on a specific complaint, a health assessment sets aside proper time to work through your health systematically, including physical measurements, relevant testing, lifestyle factors and any concerns you haven't yet raised with a doctor. The result is a documented picture of where your health stands, along with a plan for any follow-up your doctor recommends.
Medicare funds specific health assessments for eligible groups, and as a bulk billing practice, we make accessing them straightforward. Appointments are available at our Taree and Harrington clinics with easy online booking

Frequently Asked Questions
What is a health assessment at the GP?
A health assessment is a longer, structured appointment where a GP — often working with a practice nurse — systematically reviews your health rather than addressing a single complaint. It typically includes a detailed medical and family history, a review of medications and immunisations, physical measurements such as blood pressure, weight and waist circumference, screening tests appropriate to your age and risk factors and a discussion of lifestyle areas including diet, physical activity, smoking, alcohol and mental wellbeing.
The outcome is a documented picture of your current health along with a plan for any follow-up, which might involve blood tests, referrals, lifestyle changes or simply a recommended date for your next review. Health assessments differ from standard consultations in their depth and duration — many run 45 minutes to an hour — giving enough time to cover ground that shorter appointments can't.
Who is eligible for a Medicare health assessment?
Medicare funds health assessments for specific groups considered to benefit most from a structured review. These include people aged 45 to 49 who are at risk of developing a chronic disease such as type 2 diabetes or heart disease, people aged 75 and over (eligible annually), Aboriginal and Torres Strait Islander people of any age (eligible annually), people with an intellectual disability, refugees and other humanitarian entrants and former serving members of the Australian Defence Force.
A separate heart health check is also available for people aged 30 and over who haven't had a heart attack or stroke, focusing on cardiovascular risk. Eligibility rules and item numbers change from time to time, so the simplest approach is to ask your GP or practice reception whether you qualify — practices that bulk bill these assessments will confirm any costs when you book.
What happens at a 75 plus health assessment?
The over-75 health assessment is an annual, in-depth review designed to help older Australians stay healthy, independent and safely at home. Beyond standard checks such as blood pressure, heart health and medication review, it looks at the practical dimensions of daily life — mobility and falls risk, balance, hearing, vision, memory and cognition, continence, nutrition, mood and social connection.
The assessment also considers your home situation, including whether any supports or modifications would help you manage day to day, and can prompt referrals to services such as physiotherapy, podiatry or My Aged Care where appropriate. It's usually conducted partly by a practice nurse and partly by the GP, and can sometimes be arranged at home for patients unable to attend the practice. For many older patients, it's the appointment where small, manageable issues get picked up before they become the kind that threaten independence.
Assessments Matched to Your Stage
Different ages and circumstances call for different checks, which is why Medicare-funded health assessments target specific groups. Our doctors provide assessments including the 45 to 49 year old health check for people at risk of developing chronic disease, annual health assessments for patients aged 75 and over and health assessments for Aboriginal and Torres Strait Islander people of all ages.
Each assessment follows a similar structure: a detailed history, physical examination, appropriate tests and a discussion of the findings, but the focus shifts with your circumstances. A midlife check concentrates on chronic disease risk factors such as blood pressure, cholesterol, blood glucose and lifestyle, while an over-75 assessment also considers mobility, medications, memory, hearing, vision and how you're managing day to day at home.
Following your assessment, your doctor explains the findings and discusses recommended next steps, whether that's monitoring, lifestyle adjustments or further investigation.
